HAYWARD HIGH BCD BADMINTON TOURNAMENT
     Saturday-Sunday, June 22-23, 1996

SPONSORS:  
     Hayward High School Badminton &Mission Peak Badminton Supply

LOCATION:  
     Hayward High School, 1633 East Avenue, Hayward, California.  

COMPETITION FORMAT:  
     B drop C drop D flights in men’s singles and doubles,
ladies’ singles and doubles, and mixed doubles.  All entrants
begin in the B draw.  First round losers are placed in the C
draw.  First round C losers are placed in the D draw.  Single
elimination format once you have won a B or C match or are placed
in the D draw.  
     The tournament director reserves the right to cancel an
event due to insufficient entries.  If you need a partner, please
indicate so on your entry.

START TIMES:  
     Singles and mixed doubles will be held exclusively on
Saturday.  Doubles will be held on Sunday.  Matches will be
called promptly at 9 AM each day.  The gymnasium will be open at
8 AM for player registration and warm-up. Please include a
self-addressed stamped envelope if you would like a personalized
start time or directions to the gymnasium.  
     A 5 minute default will be strictly enforced.  Please arrive
at least 20 minutes before your first match to check-in at the
tournament desk.  

ENTRIES:  
     $8.00 donation per event.  
     Please make checks payable to: Irfan Shahab.  
     Entries and payment must be received by June 15.  Phone-in
and late entries will be charged an additional $4.00 per event
and will be accepted at the discretion of the tournament
director.  Entries that do not include payment will not be
accepted.  Absolutely no refunds after the draw is made.

AWARDS:  
     Finalists of “B” events and winners of “C” & “D” events will
receive prizes.

WAIVERS:  
     All players must sign the accompanying waiver before being
allowed to compete.  Minors under the age of 18 must have the
waiver signed a parent or legal guardian.

Waiver:  By signing below, I hereby release, waive, discharge and
covenant not to sue the Hayward High BCD Badminton Tournament,
the promoters, other participants, operators, officials and
sponsors of the event, herein referred to as “releasees” from all
liability to the undersigned, his/her personal representatives,
assigns, heirs and next of kin for any and all damage and any
claim or demand therefor on account of injury to the person or
property or resulting in death of the undersigned, whether caused
by the negligence of the releasees of otherwise while the
undersigned is competing, officiating in, observing or working
for, or for any other purpose participating in the event.  I
hereby agree to indemnify and save and hold harmless the
releasees from any loss, liability, damage or cost I may incur. 
I hereby assume full responsibility for and risk of bodily
injury, death or property damage.
     The undersigned has read and voluntarily signs the release and
waiver of liability and indemnity agreement.
