Events:             BS, GS, BD, GD, and MX in the following age categories:

               U-22 (Born in 1981 or later), U-19 (Born in 1984 or later), U-17 (Born in 1986 or later)

U-15 (Born in 1988 or later), U-13 (Born in 1990 or later), U-11 (Born in 1992 or later)

                        U-9 (Born in 1994 or later)

                        Maximum 3 events per player. (Player’s age must be under the age category as of 12/31/02.)

Entry Deadline:    Tuesday,  26 february 2002.  LATE ENTRIES WILL NOT BE ACCEPTED.

 

Entry Fees:     $15 per person for the first event. $5.00 per person per event for the second and third events.

All participants must be members of USA Badminton. (One year USAB Junior memberships can be obtained for $15.00 at the registration desk). Make checks payable to USBDF.

 

Awards:            Medals will be awarded to champion, runner-up, and consolation winner.

 

Conditions:

1.      Events may be combined or played round-robin format if not enough entries are received.

2.      Consolation draws will be held for each event.

3.      Up to two Yonex feather shuttlecocks per match will be provided free of charge.

4.      All matches will be played in the best of 5 games to 7 points format.

5.      A player not ready to play ten minutes after his/her match is called will be defaulted.

6.      Each player in a doubles partnership must submit separate entry forms.

7.      Entries will not be refunded if cancelled after 5 March 2002.

8.      Results will be submitted for ranking purposes.

9.      The Tournament Director reserves the right to change any of the above conditions.

USA BADMINTON WAIVER AND RELEASE OF LIABILITY

NOTE: THIS FORM MUST BE READ AND SIGNED BEFORE PARTICIPANT IS PERMITTED TO TAKE PART IN EVENT SESSIONS.  BY SIGNING THIS AGREEMENT, PARTICIPANT

             AFFIRMS HAVING READ IT.

IN CONSIDERATION of my involvement in the 2001 USBDF SO-CAL Junior Open badminton tournament sponsored by the US Badminton Development Foundation and the Orange County Badminton Club (OCBC), Orange, CA, sanctioned by USA Badminton, I acknowledge, appreciate and agree that:

 

1.     I risk bodily injury, including paralysis, dismemberment, disability and death, and while particular rules of the sport, equipment, and discipline may reduce this risk, this risk of injury does exist, as well as the risk of damage to or loss of property;

2.     I knowingly and freely assume all such risk, both known and unknown, even if arising from the negligence of the releasees or others;

3.     I willingly agree to comply with the stated and customary terms and conditions for participation.  If, however, I observe any unusual or unnecessary hazard during my presence or participation or if I observe any concern in my readiness for participation, I will immediately bring such to the attention of the nearest official and refrain from participation; and,

4.     I, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, hold harmless and promise not to sue USA Badminton, the2001 USBDF SO-CAL Junior Open badminton tournament committee, the OCBC, their officers, volunteers, staff, sponsors and/or agents, (“releasees”) with respect to any and all injury and loss arising from my participation, whether caused by the negligence of the releasees, the condition of the premises or otherwise, except that which is  the result of gross negligence or wanton misconduct, to the fullest extent permitted by law;

5.     I agree to be bound by the rules and regulations of USA Badminton and I hereby stipulate that I am eligible to play in the events for which I am applying and that I understand that the above mentioned make no representation or warranty with respect to the condition of the premises or the operation of the event;

6.     I, hereby grant to USA Badminton, its licensees and contractees including photographers, televisions and motion picture companies, their affiliates and subsidiaries, full television and motion picture rights including authority to film or videotape me during matches, narratives, personal interviews, or comment thereon for any and all commercial, news or other purposes together with the right to transfer or grant their rights to others, all without remuneration or compensation to me whatsoever.

I have read this Release of Liability and Waiver Agreement, fully understand its terms, understand that I have given up substantial

rights by signing it, and sign it freely and voluntarily without any inducement.
