CAL STATE HAYWARD BADMINTON TOURNAMENT I
		 July 15 & 16, 1995 (CSUH GYMNASIUM)
			  Hayward, CA 94542

EVENTS:	
	Open, C and D flights in all events.  The tournament committee 
	reserves the right to make any necessary changes which includes 
	combining flights.  PARTICIPANTS MAY NOT PLAY IN THE SAME EVENT IN
	MULTIPLE FLIGHTS.  MAXIMUM OF THREE EVENTS PER PERSON.

SANCTION:
	Open and C flights are sanctioned by CSUH, Associated Students and 
	USBA.  All Open and C players must show their current USBA ID cards
	at the registration desk or pay a $5 tournament participation fee.

DEADLINE:
	All entries must be postmarked by July 5, 1995.  All late entries will
	be charged an additional $2 per person.  One entry form per player.
	Entry form must accompany fees.

ENTRY FEES:
	1 event: $10		2 events: $19		3 events: $28
	No phone-in entries accepted.

TIMES:
	Play begins at:	Saturday:	Singles 8:45AM	Mixed: (approx) 1:00PM
			Sunday:		Doubles 8:45AM
	15 min. default rule strictly enforced.
	Send a self-addressed stamped envelope/postcard or include your email
	address with this entry form for individual starting times.

MISC:
	Official Birds: HL Shuttle, Yonex or HiQua (Available at the tournament
	desk for $1.60 each).
	Awards will be given to the 1st and 2nd place and consolation winners.
	NO FOOD OR DRINKS ALLOWED IN THE GYM.

may arise from my participation in this tournament.  In consideration of my
being permitted to participate in this competition.  I, the person named below,
release and discharge the USBA, Associated Students, Cal State Hayward, Cal
State University, their officers, members, agents, and administrators any and
all participants in this event harmless for any bodily injury to myself and
others, or for any damage to, or loss of my property during the course of the
event.  I HAVE READ THIS AGREEMENT, UNDERSTAND ITS PURPOSES AND AGREE TO ITS
